Supplementary application forms
Some schools within Dorset Local Authority require applicants to fill in forms in addition to the standard forms.
If one of your choices is listed below you need to fill in a supplementary form. This is in addition to the application process described in 'How to apply for a school place' (both paper based and online).
If making a paper application please return the supplementary form with your application according to the instructions on the form. If making an online application please return the supplementary form to the School Admissions Team County Hall, Colliton Park, Dorchester, Dorset DT1 1XJ.
