Community Learning Disabilities Teams
Our Community Learning Disabilities teams (CLDT) are the first point of contact for advice, information and guidance on services for people with a learning disability.
Who are we?
We are a team of social care, health professionals and Community Care Officers who help adults with a Learning Disability.
There are three integrated teams:
- East (Wareham/Ferndown/Christchurch)
- West (Bridport/Sturminster Newton)
- Weymouth and Portland
Who uses our service?
We offer a range of services which we will talk about with you and if you are happy, we will also talk with your family, carers and/or advocate. These services include:
- Assessment
- Advice and information
- Individual treatment/therapy
- Monitoring of mental and physical health and social needs
- Support and advice for medical care
- Referrals to other health/social care groups
- Training for staff/carers

- Speech and Language Therapists
- Approved Mental Health Practitioners (AMHP)
Who can receive our service?
The Community Learning Disabilities Teams work with people who have a learning disability, who are 18 years and over and originate from within the Dorset County Council area or are registered with a Dorset GP.
How to access our services
Anyone who knows you can refer you to our team. This includes your GP, your Social Worker, your advocate, carer or your family. You can also refer yourself either in writing or by telephone. You can make a referral by calling the Adult Access Team on the number below.
What happens once you are referred?
A member of our team will come and visit you and get some information about what you need help with. They will then come back to our team meeting and we will talk about who is the best person to work with you. Please note we may need to carry out a financial assessment and charge for some services.
If we are unable to help you, we will tell you who might be able to instead and let those people know what you need help with.
