Hospitalisation - help returning home
Adult and Community Services social work teams are based within the Dorset County, Poole, Royal Bournemouth and Christchurch NHS Foundation Trust Hospitals.
We work with adults who are Dorset residents and in hospital and who may need advice or assistance to achieve a safe and timely discharge.
We are committed to focusing on service users and their carers and will carry out a community care assessment of needs and if necessary, will set up a package of care to meet eligible needs and assist them after discharge. We work closely with various members of the multidisciplinary hospital teams.
In the Dorset County and the Royal Bournemouth Hospital, we work alongside
Help and Care (opens in a new window) 
who offer simple advice and guidance to patients. Help and Care may identify individuals requiring referral to the Adult and Community Services team for fuller assessment.
