Lead professional and team around the child
Lead professionals work with children and young people with additional needs who require an integrated package of support from more than one practitioner. It is usual for the lead professional to be supported by a Team Around the Child (TAC).
The lead professional is responsible for coordinating the actions identified through the assessment process. They are also a single point of contact for children, young people with additional needs, and their families.
The team around the child is made up of professionals across the children's workforce, convened together to meet the needs of the child or young person.
Lead professionals can access Locality Funding, held by their Locality Manager to support actions identified through a common assessment. To access this fund a lead professional would be required to complete a

and send it to their Locality Manager..
