Disabled leisure pass
If you are registered disabled or long term sick you may be entitled to discounted entry into West Dorset & Weymouth leisure centres.
How to apply

What you need to do
Complete the form and return it in person (do not post) to one of the registration points below, together with two black and white or colour passport-sized photographs, proof of identity, residency (i.e. driver's licence, household bill) and proof of benefit received. You will then be issued, by post, with a numbered leisure pass. The pass is valid for 12 months, after which time, if there is no change of circumstances, you may re-apply.
