How to apply

Full details of who can apply and an application form can be found on the right of this page or from the leisure centres, West Dorset tourist information centres and the Leisure & Tourism Division at Stratton House, High West Street, Dorchester, Dorset  DT1 1UZ.

What you need to do

Complete the form and return it in person (do not post) to one of the registration points below, together with two black and white or colour passport-sized photographs, proof of identity, residency (i.e. driver's licence, household bill) and proof of benefit received. You will then be issued, by post, with a numbered leisure pass.  The pass is valid for 12 months, after which time, if there is no change of circumstances, you may re-apply.
